At Bakersfield Memorial Hospital, we believe admission should be a simple, quick procedure. Step one begins with your doctor making a reservation for you. If the reservation is made a few days before your scheduled admission or surgery, you may pre-register to make your admission faster on the day you arrive. Before you are admitted or receive outpatient services at the hospital, you will need to complete the registration process.
We want your admission experience to be a positive one. To make the process easier, the Preadmission Registration Form (pdf) is now available in PDF format. Please print, fill out, and mail or fax to BMH Admitting Department (contact information below). If you would prefer, you may pre-register in person or by telephone. If you have not signed your registration papers by the day of service, plan to arrive early in order to complete the registration process. For your convenience, the Admitting Department is located off of the main lobby on the first floor, just passed the front desk.
Labor and Delivery patients should pre-register after 24 weeks gestation.

What to Bring:
- Your hospital insurance card(s)
- Driver's license/photo ID
- Orders or forms from your doctor
- Any deductibles and co-pays will be collected at time of admission or prior to discharge. Payment may be made by cash, check or major credit card. You may wish to contact your insurance provider to determine whether you have a deductible or co-pay amount and how much it will be.
- If you have completed an advance health care directive form, please bring a copy with you.
- A list of your current medications (unless information has already been reported)
- For your comfort, we suggest that you bring a pair of pajamas or a nightgown, slippers, robe, and toiletries (such as shampoo and toothbrush), etc.
- Don't forget your glasses, hearing aids and dentures if needed
- For patients under age 18, a parent or guardian must sign consent forms. Guardians must present their copy of the court order granting their guardianship of the minor when registering and on the day of surgery.

- To ensure your safety, you may not bring any electrical items that operate on household voltage (such as hairdryers, curling irons, hot rollers, radios, computers, VCRs, razors and DVD players.) Some personal appliances are available for loan. Ask your nurse for more information
* If you need to bring cash, checks or a credit card to make a deposit or co-payment, please have a family member take them home after you are admitted.
Insurance:
Many insurance carriers require the patient to be responsible for a co-payment or percentage of approved charges. Co-payments and deductibles are due at the time of service. Payment may be made by cash, check or major credit card. Because insurance requirements vary by contract, it is the patient's responsibility to verify that our hospital is a contracting provider and that all requirements are met for authorization and notification. Our Admitting staff will assist in any way possible.
