Transition from hospital to home
As our patients begin their path to recovery, there are times with their physical condition is stable and the intensity of nursing care provided on our acute patient care units is no longer necessary. However, some patients are still not quite well enough to be safely discharged. Additional treatment or healing time may still be necessary due to the nature of their illness.
In this case, patients may be transferred to our Skilled Nursing Unit before a final discharge home. The Skilled Nursing Unit serves as a transition point where a patient receives additional specialized support services or therapy in preparation for going home. Our team of nurses, assistants, therapists and others will help to make this transition as smooth as possible. The supportive team also assists in easing the discharge home.
