Mercy and Memorial Hospitals Home Health Services provide total care for patients who have recently been discharged from the hospital or homebound individuals needing short-term, skilled nursing, or therapeutic care.
Our Home Health Services provide quality care, directed by a physician, done in a safe and professional manner in the comfort of the recipient's own home. By coordinating care with the physician, they provide that important link in the continuum of care between hospitalization and the return to everyday life. They are able to meet each patient's individualized needs, with the most qualified nursing personnel available. Patients may be referred by physicians, discharge planners, private insurance companies, clients, community facilities and other public health service providers.
Mercy and Memorial Hospitals Home Health Services provide the following services: Skilled Nursing, Physician Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, and Home Health Aide.

HOURS: Office Hours - Monday through Friday from 8:00am-5:00pm
An on-call RN is available 24 hours a day, seven days a week.
